- Please complete this form with any additional attachments
- Bring all completed forms to the registration table along with $155.00 per person staying on-site and $75.00 per person staying off-site $8.50 additional for meals (commuters) to complete the registration process.
- Monthly Contributor's registration guidelines will apply.
- Attire for the Youth Convention is "Christian Casual".
IMPORTANT: Please adhere to the age guidelines this convention is for youth between the ages of 16 - 26 years of age: Youth ages 16 to 18; College Students age 19 to 26). All attendees "must" register, registration is per person.
